Disability insurance may not be a top priority to you at this point in your career.  The problem with a disability is that it will not wait for the right time to happen.  You run the risk of becoming uninsurable during the time you are finishing your residnecy. This is why you should get coverage as soon as possible.  You may have some coverage provided by your school or hospital that you are affiliated with.  The advantage you have is that insurance companies will allow you to have "special limits" of coverage which can sometimes be more than you are currently earning.  This is due to the training you have had so far being very valuble in your career so they allow these limits.  You should consider taking a small amount of coverage out with a rider that will allow you to increase the policy once your income starts to increase.  This rider helps you to eliminate the medical underwriting and will only be the financial insurability.  This will allow you to increase the policy no matter what your health condition is at that time as long as your income is sufficient to increase the benefit.
